How to Complete the Form

  • Provide your contact details and your pet’s information, including name, species, and procedure type.
  • List current medications and note any recent changes in your pet’s behavior, appetite, or activity.
  • Acknowledge pre-surgical instructions, such as fasting or medication withholding.
  • Submit the form online so our veterinary team can review it before your visit.

This information allows our veterinarians to tailor anesthesia, pain management, and care protocols specifically to your pet’s needs.

This form ensures our team has the most accurate and up-to-date information about your pet’s health, recent medications, and any special considerations. By filling the document out in advance, you’ll save time at check-in and help us focus on what matters most — your pet’s safety and comfort.

Procedures requiring anesthesia are always associated with a certain amount of risk. We want to minimize that risk as much as possible - this requires a physical exam (if the pet has not been seen within the past six months). General anesthesia has become safer in recent years with the advent of newer drugs and better patient monitoring. Some conditions may not, however be evident on a physical exam. To better ensure your pet's safety during anesthesia, we advise the following pre-anesthetic tests be performed, even for elective procedures such as spays, declaws, dentals, lump removals and castrations.
