Before your pet’s dental cleaning or oral treatment, please complete our required Dental Check-In Form online.

How to Complete the Form

  • Enter your contact information and your pet’s details, including name, species, and age.
  • List all current medications and note any new health concerns or changes in eating habits.
  • Review and confirm pre-dental instructions, such as fasting or medication withholding.
  • Submit the form online so our team can review it before your pet’s scheduled procedure.

Dental Check-In Form

This form helps our veterinary team gather essential information about your pet’s current health, medications, and recent behavior changes. Submitting it ahead of time allows us to streamline your pet’s check-in and begin their dental care without delay.

Procedures requiring anesthesia are always associated with a certain amount of risk. We want to minimize that risk as much as possible - this requires a physical exam (if the pet has not been seen within the past six months). General anesthesia has become safer in recent years with the advent of newer drugs and better patient monitoring. Some conditions may not, however, be evident on a physical exam. To better ensure your pet's safety during anesthesia, we advise the following pre-anesthetic tests be performed.

Possible additional costs: After an oral examination under anesthesia, we may recommend dental radiographs and/or extractions. The cost for each extraction varies depending on which tooth is affected and the level of disease. Pets in need of such additional treatment can further costs $100-$400 beyond the cost of the cleaning, depending on the severity of oral disease. Oral pain medications and antibiotics will be sent home following most extractions.
